Please note, we are only looking to proceed with candidates who have full-time availability and meet the scheduling requirements below.</strong></p><p></p><p><strong>FT Scheduling Requirements:</strong></p><ul><li> Available for 10-12 hour shifts</li></ul><ul><li>Available <strong>Monday-Friday, day and evening availability preferred</strong></li><li><strong>MUST </strong>be able to commit to a <strong>consistent, ongoing, weekly schedule</strong> for the foreseeable future</li><li>available and willing to see multiple clients throughout the day, your day will likely consist of seeing multiple clients throughout this region. </li></ul><p><strong>PT Scheduling Requirements:</strong></p><ul><li>Available for 10-12 hour shifts</li></ul><ul><li><strong>MUST</strong> be available <strong>Saturday AND Sunday, day and evening availability preferred</strong></li><li><strong>MUST </strong>be able to commit to a <strong>consistent, ongoing, weekly schedule</strong> for the foreseeable future</li><li>available and willing to see multiple clients throughout the day, your day will likely consist of seeing multiple clients throughout this region.</li></ul><p><strong>Requirements</strong></p><ul><li><strong>Previous experience supporting Ontario Health atHome (formly HCCSS) client is a strong asset</strong></li><li><strong>Previous experience working as a visiting nurse in the community is a strong asset</strong></li><li>Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs</li><li>When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions. </li><li>Support the client through care transitions by collaborating and connecting the client with community support services in their local community.</li><li>Administer medications orally, topically, subcutaneously, or via G-tube, as per CNO scope and documented medical orders; document all administrations in the electronic Medication Administration Record (eMAR).</li><li>Monitor and assess clients’ vital signs (blood pressure, pulse, temperature, respirations, oxygen saturation), document findings, and report any abnormal readings or concerns to the designated Care Coordinator or physician.</li><li>Support clients with medical needs such as oxygen therapy (monitoring levels, changing tanks, checking saturation), G-tube feeding (preparing and administering formula, flushing lines), and suctioning as per training and client-specific protocols.</li><li>Safely assist clients with mobility and transfers using mechanical lifts (Hoyer, sit-to-stand) or manual assistance, following prescribed mobility plans and ensuring proper body mechanics and safety procedures.</li><li>Perform prescribed range of motion (ROM) and physical therapy exercises under direction of a physiotherapist or care plan, and assist with ambulation inside the home or outdoors if required.</li><li>Observe and document any changes in the client’s physical, cognitive, or emotional condition, including changes in skin integrity, alertness, behaviour, and breathing patterns; escalate concerns as needed.</li><li>Maintain accurate and timely documentation of all care provided, including progress notes, incident reports, and communication logs, in compliance with company and regulatory standards.</li><li>Use and dispose of Personal Protective Equipment (PPE) such as gloves, masks, gowns, and eye protection as per infection prevention and control protocols and Public Health Ontario guidelines.</li><li>Comply with organizational policies regarding infection control, client safety, privacy, and professional conduct; report all hazards or incidents to the supervisor immediately.</li></ul><p><strong>The following Requirements are MANDATORY:</strong></p><p></p><ul><li><strong>Active registration</strong> and in good standing with the <strong>College of Nurses of Ontario (CNO)</strong> as a Registered Nurse (RN)</li></ul><ul><li><strong>Current CPR and First Aid certification</strong>, completed through a recognized training provider within the past 12 months.</li><li><strong>Valid WHMIS 2015 certification</strong>, issued or renewed within the past 12 months.</li><li><strong>Negative TB test</strong> (2-step or 1-step with prior documentation), completed within the past 12 months.</li><li><strong>Proof of full COVID-19 vaccination</strong>, or exemption in accordance with the Ontario Human Rights Code.</li><li><strong>Clear Criminal Record Check with Vulnerable Sector Screening</strong>, issued within the past 12 months.</li><li>Must be able to <strong>communicate effectively in English</strong>, both orally and in writing, for documentation and communication with clients and care teams.</li><li><strong>Must be comfortable and capable of providing care to both male and female clients</strong> of varying ages and care needs.</li><li><strong>Must have a valid driver's licence and reliable access to vehicle to complete daily tasks</strong></li><li>Ability to identify and report client care concerns to Care Coordinators when needed.</li><li>Demonstrated <strong>experience delivering in-home care/community settings</strong></li><li>Gerontology competencies preferred</li><li>Ability to be flexible and adaptable with excellent organizational skills</li><li>Ability to take initiative and adapt to rapidly changing demands</li><li>Excellent interpersonal, communication (oral and written) and critical thinking skills</li><li>Demonstrated skills working independently and in a team environment</li></ul><p></p><p></p><p><strong>Benefits</strong></p><ul><li>Competitive rates of pay $35-$45 depending on nursing designation</li><li>Milage Reimbursement</li><li>Performance-based bonuses</li><li>PPE provided</li><li>24/7 Nursing Oversight</li><li>Educational Training Opportunities</li><li>Employee Recognition Program</li><li>Health and Wellness Program</li></ul></p></p>Create Your Resume First
