<h3>Job Description</h3><p>Job Description<p>Salary: $40-$45/hr<br /><p>Social Worker / Social Service Worker</p><p><br><br></p><p>Position Summary:</p><p></p><p>As a Social Worker / Social Services Worker, you will work as part of an interdisciplinary team, reporting to the Executive Director / Administrator in the Home. As the Social Worker, you will support / assist residents with managing their overall well-being. You will advocate for the resident as requested, respecting their rights under the Resident Bill of Rights. As a member of the interdisciplinary team, you will support the residents and family members in understanding their plan of care. You will assist to achieve and maintain occupancy targets.</p><p></p><p>Key Responsibilities:</p><ul><li>Facilitate the admission process in the home and completes all occupancy tracking.</li><li>Facilitate internal room transfers.</li><li>Facilitate discharges from the home.</li><li>Lead the rate reduction application process for residents annually.</li><li>Coordinate family education sessions (virtually and in-person) as requested.</li><li>Act in an advocacy role as appropriate to support residents.</li></ul><ul><li>Provide counselling support to residents and family members.</li><li>Conduct tours of the home to potential resident and family members.</li><li>Coordinate and maintain a good relationship with outside agencies and stakeholders.</li><li>Organize and attend interdisciplinary care conferences including special care conferences.</li><li>Participate in Continuous Quality Improvement initiatives in the home.</li></ul><ul><li>Complete program evaluations and assigned audits.</li><li>Chair of the Ethics Committee in the home, if assigned.</li></ul><ul><li>Participate on the Palliative Care Team / Committee as assigned.</li><li>Prepare all admission packages for the home.</li><li>Act as a resource to the internal Behavioural Supports Ontario team.</li><li>Comply with Plan As guiding principles, all regulatory requirements, and respect the Resident Bill of Rights.</li><li>Meet with Resident Council and Family Council monthly, as requested.</li><li>Other duties as assigned.</li></ul><p></p><p>Requirements:</p><ul><li>Bachelor of Social Work Degree from a recognized university OR Social Services Worker Diploma from a recognized college.</li><li>Current membership with the Ontario College of Social Workers and Social Service Workers required, and in good standing.</li></ul><ul><li>Minimum of 2 years experience in the health care sector</li><li>Bilingualism is an asset.</li><li>Flexible with working hours to provide evening support as required.</li><li>Palliative Care experience an asset.</li><li>Knowledge of Behaviour Supports Ontario an asset.</li><li>Knowledge of Occupational Health & Safety practices, principles, and legislation</li><li>Excellent active listening skills.</li><li>Empathetic, with a passion and focus on the person-centered approach.</li><li>Strong problem-solving, negotiation and conflict resolution skills, with the ability to foster collaboration, teamwork, and the ability to work with a diverse population.</li></ul><ul><li>Excellent teamwork and team-building skills.</li><li>A high degree of resourcefulness, flexibility, and adaptability.</li><li>Able to effectively communicate verbally and in writing.</li><li>Politically and culturally sensitive.</li><li>Committed to staff and resident privacy.</li><li>Tuberculosis testing within the last two years; Steps 1 & 2; or an X-Ray from the previous five years showing a clear result.</li></ul><ul><li>Able to provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.</li><li>Proof of current immunization in accordance with Plan A policy.</li></ul></p></p>Create Your Resume First
