<h3>Job Description</h3><p>Job Description<p><p><strong>Transitional Case Manager, PEPP (Prevention and Early Intervention Program for Psychoses)</strong><br /><em>Direct Service - (OPSEU)</em><br />Full-time, Permanent (Benefitted)<br />St. Thomas, ON (Elgin Zone)<br /><strong>- Application Deadline: September 22, 2025 -</strong></p><p>The Transitional Case Management, PEPP, provides comprehensive and flexible case management services, offering opportunities and support for individuals with first episode psychosis to experience a variety of life situations, which further develop levels of independence, and promote the achievement of client self-prescribed goals. They also offer direct treatment intervention such as counselling and access to medical consultations for appropriate medication. </p><p><em>Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. For candidates who require a request letter in order to apply for a police check, request letters will be provided for candidates who move forward from the interview stage of the recruitment process. </em></p><p><strong>Consider revitalizing your career and making a difference in your community by joining our team!</strong></p><p><strong> </strong></p><p><strong>WHAT WE'RE OFFERING:</strong></p><ul><li><b>What you'll be making - </b>Starting rate for this role is <b>$27.14</b> per hour (Level 1 – Direct Service), with a wage differential, based on regulation (<b>$30.01</b> per hour for Level 1 – Direct Service, Regulated):<ul><li>Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation</li><li>Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)</li><li>For internal applicants, grid level placement will occur as per the Collective Agreement</li></ul></li><li><strong>When you'll work </strong>- 70 hours over a two-week period; Monday to Friday, 8:30am to 4:30pm</li><li><strong>What your benefits will be </strong>- After a 90-day waiting period, will be enrolled in the extended healthcare benefits plan</li><li><strong>Your pension plan</strong> – Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)<br /> </li></ul><p><strong>HOW YOU WILL BE MAKING A DIFFERENCE:</strong></p><b><i>Identify individuals suitable for PEPP case management:</i></b><ul><li>Proces referrals from inpatient treatment units within St. Thomas - Elgin General Hospital (STEGH) Mental Health Care and Regional Mental Health Care, London and referrals from other service providers, public health, and other community agencies, and as self-referral in conjunction with team</li><li>Actively seek out persons with the community who might require or benefit from PEPP case management services and encourage program involvement</li><li>Ensure effective community linkage by considering and coordinating all input from team and referral sources</li></ul><b><i>Assess consumer rehabilitative needs:</i></b><ul><li>When appropriate, obtaining and evaluating information from consumer and members of their social network, referral source, etc. regarding client's psychiatric history, level of social functioning, <span >behavioural/medical</span> problems, special living/learning disabilities, residential, financial and vocational status, etc., to ensure appropriate supports</li><li>Provide direct service by such interventions e.g. counselling, access medications assuring consumers linkage to further specialized assessment as required</li><li>Consultation with PEPP Nurse and Psychiatrist and community service providers to meet the needs of PEPP clients in their recovery</li><li>Present individualized program plans at weekly team meetings for discussions and problem solving/joint care programming with other members of the team</li></ul><b><i>Formulate and implement rehabilitation plan and provide ongoing community support:</i></b><ul><li>Develop a treatment plan in collaboration with consumer and other direct service providers</li><li>Health teaching with PEPP clients and their family members</li><li>Assess consumers in dealing with environmental difficulties and to develop social living skills conducive to community living </li><li>Advocate on behalf of consumers in relation to hospital and community services, landlords, police, etc. to ensure consumers rights are not compromised</li><li>Establish and maintain liaisons and co-operative working relationships with local community social agencies, boarding home operators, etc. promoting understanding and positive attitudes towards the special needs of those dealing with First Episode Psychosis and provide education and support to various community representatives</li></ul> <p><strong>WHAT YOU'LL NEED TO APPLY:</strong></p><p><strong><em>Police Vulnerable Sector Check Requirements</em></strong></p><p>Successful candidates must have a Police Vulnerable Sector Check completed in the last six months.
