<p><p>Manager Medication Access – Montefiore Health System – Yonkers, New York – Pay: $76,000.00-$95,000.00 – Full‑time – Day shift – MON‑FRI 8:30 AM‑5:00 PM (7.5 hrs/day)</p><h3>Overview</h3>
<p>The Medication Access Manager leads initiatives dedicated to ensuring continuity of medication access and optimizing pharmacy‑related processes for both inpatient and outpatient care. The role oversees daily operations, guides strategic initiatives to reduce avoidable readmissions, and enhances patient engagement and medication adherence for high‑risk populations.</p>
<h3>Essential Responsibilities</h3>
<ul>
<li>Supervise and mentor a team of Pharmacy Specialists, providing guidance, performance feedback, and professional development.</li>
<li>Monitor workload distribution, productivity metrics, and service quality to ensure efficient and patient‑centered operations.</li>
<li>Lead departmental initiatives focused on improving medication access, care transitions, and readmission prevention.</li>
<li>Develop and implement standard operating procedures and best practices for outpatient medication coordination.</li>
<li>Prioritize customer service by consistently demonstrating professionalism, empathy, and respect in interactions with patients, visitors, and colleagues.</li>
<li>Utilize established clinical criteria to identify patients at elevated risk for hospitalization and ensure timely access to essential medications.</li>
<li>Perform detailed reviews of home medication lists through patient interviews and direct engagement with outpatient pharmacies to ensure accuracy and continuity of care.</li>
<li>Maintain detailed records of patients whose discharge medications are billed to Montefiore, tracking associated 30‑day readmission rates.</li>
<li>Lead efforts to identify and eliminate obstacles to prescription fulfillment by coordinating with community pharmacies, patient assistance programs, third‑party payers, and internal care teams.</li>
<li>Engage patients in meaningful dialogue to uncover and address any additional barriers to medication access.</li>
<li>Oversee the implementation of bedside medication delivery programs where applicable.</li>
<li>Partner with interdisciplinary teams—including pharmacy, nursing, case management, and social work—to identify and resolve systemic barriers to medication access.</li>
<li>Identify opportunities for process enhancement and deliver targeted in‑services to interdisciplinary teams.</li>
<li>Represent the department in cross‑functional meetings and committees focused on care transitions, health equity, and patient outcomes.</li>
<li>Collaborate with external stakeholders such as community pharmacies, third‑party payers, and patient assistance programs to streamline prescription fulfillment.</li>
<li>Oversee the completion of prior authorizations, insurance verifications, and enrollment in patient assistance programs.</li>
<li>Actively participate in departmental meetings and quality improvement projects as assigned.</li>
<li>Maintain comprehensive records of all clinical interventions, adverse drug events, and medication errors.</li>
<li>Ensure timely and accurate documentation of patient interactions and interventions.</li>
<li>Analyze trends in medication access challenges and develop targeted solutions.</li>
<li>Maintain records and generate reports on key performance indicators, including prescription costs billed to Montefiore and 30‑day readmission rates.</li>
<li>Facilitate training and in‑services for staff and clinical partners on medication access workflows.</li>
<li>Lead community outreach efforts to promote health literacy and medication adherence.</li>
<li>Support bedside medication delivery initiatives and align with discharge planning protocols.</li>
</ul>
<h3>Experience Requirements</h3>
<ul>
<li>Minimum of 3–5 years of experience in outpatient pharmacy, care coordination, or medication access roles.</li>
<li>At least two years of experience in a retail/community pharmacy setting with knowledge of third‑party payer reimbursement and medication prior authorization processes.</li>
<li>At least one year of experience in a hospital or clinical setting, with familiarity in discharge or care transition workflows.</li>
<li>Preferred: 1–2 years of supervisory or team‑leadership experience.</li>
<li>Bilingual in English and Spanish (oral and written) required.</li>
<li>Proficiency in Microsoft Office applications and general technology use.</li>
</ul>
<h3>Education</h3>
<ul>
<li>Bachelor’s degree in Pharmacy, Healthcare Administration, Public Health, or related field required. Advanced degree preferred.</li>
</ul>
<h3>Licensure & Certification</h3>
<ul>
<li>Active Pharmacy Technician Certification (PTCB or equivalent national credential)</li>
<li>NYS Pharmacy Technician license and registration required.</li>
</ul>
